At The Luxe Medspa, our goal is to provide our patients with safe, individualized care that delivers results. To achieve this, we walk each patient through our structured intake process.

STEP 1: Intake

In Georgia, all patients receiving Botox, Dermal Fillers, Laser Treatments, and/or Medical Grade Skincare treatments are required to receive a Good Faith Exam to determine their eligibility for treatment. At The Luxe Medspa, our priority is your safety. Before beginning this exciting journey, we need to complete your intake process.

Virtual Good Faith Intake Questionnaire.

The Virtual Good Faith Intake Questionnaire can be completed online from your phone or computer and typically takes 5-10 minutes to complete. We will obtain basic contact information and also pertinent medical information such as past medical history, past/current medications, and previous procedures. This will also help our medical provider ensure your safe transition into the treatment phase.

After completion of your Intake Questionnaire, you will be redirected to the scheduling page to schedule a time to go over your Intake Questionnaire with our Board Certified Provider via televisit for clearance. Televits can be completed via phone or computer and typically require 10 minutes to complete.

STEP 2: Consultation | In-Office Treatment Scheduling

Aesthetic, Skincare, and Laser treatments are not one size fits all. For your convenience, consultations will take place via televisit with our Aesthetic Nurse. During this time we will review your treatment goals and questions, develop a care plan tailored to your needs, review consent forms, and schedule you for your in-office treatment(s). Consent forms must be completed before your in-office appointment to prevent cancellation.

SERVICES

If you have not been seen in the office in the past 4 months and would like to receive treatments OTHER THAN BOTOX|XEOMIN, LIP FILLERS, OR HYDRAFACIAL please schedule a CURRENT PATIENT - VIRTUAL CONSULTATION. Things change with time. The virtual appointment will allow our Aesthetic Nurse to make the proper adjustments to your care plan, as well as answer any questions you may have about your treatment(s).

GOOD FAITH EXAMS MUST BE COMPLETED BY ALL PATIENTS PRIOR TO IN-OFFICE VISITS TO PREVENT CANCELLATIONS
